FileUpload
The 'FileUpload' question type provides a location for your respondent to upload a file with their form entry. The file will be uploaded to the server which can then be viewed or downloaded by the account owner.
You can add up to five file upload questions to any single form. Once this quota has been reached, you will be unable to select a new file upload question.
The respondent selected file must be formatted to one of the following format types for the upload file to comply (the form creator will set what file types can be uploaded whilst creating this question type):
- Image Files ( .jpg, .jpeg, .pjpeg, .png, .gif )
- Document Files ( .doc, .docx, .pdf, .msg, .txt )
- Spreadsheets ( .xls, .xlsx, .csv )
- PowerPoint File ( .ppt, .pptx )
- AutoCad File ( .dxf )
- Zip File ( .zip )
There is a maximum file size of 5MB permitted for upload. If you require the respondent to upload or send a larger file with their entry, provide your any contact details in the 'Completion Message' or include your email in the 'Footer' section for additional contact.
Note! We highly recommend that when creating a form, you situate the 'File Upload' question type on the last page of a multi-page form.

Use Cases
Additional Documents
When your form requires additional documents from the respondent that cannot be re-written using one of the question types, you should include a 'FileUpload' question type at the end of your form.
This can be useful if the form requires passport information, health documents or legal documents from the respondent.
Valid ID
Proof of valid ID can be obtained using this feature, as the respondent can upload an image of their in hand identification card. Valid in-hand identification could be a passport, drivers license or health card.
If more than one valid ID is needed, you can add multiple 'FileUpload' question types in order for your respondent to show proof.
